Can someone explain to me how one progresses from fighter to Crusader to Shadow Knight?

This is probably not what you are expecting to hear but if its anything like the lvl 10 hallmark quest, your hallmark quest will probably update as you get closer to lvl 20. Your journal will then have additional details.

Hmm. In my haste I realized that I didn't answer your question very well.
Basically around lvl 8 or 9 your quest journal updates. You will see additional details in your Hallmark quest entry (in quest journal). Read this carefully and you will know where to go to become a crusader.
I'm assuming the process is similiar when you are close to lvl 20. Good luck.

You can actually start your class (and subclass) quests a level or two early if you go "hail" the proper NPC, but they will appear automatically in your journal at level 9 (and 19) if you don't do this.

You can start your class (SK) quest at level 18 and finish it if you can. When you ding level 20 you get your class and title. I was grouped with a shaman at level 19 that did his quest already and he dinged, bam a defiler. Hope that helps.
