PoP Level Requirements

What are the level requirements for certian PoP zones? i know PoV is 55 and BoT is 62 but what are the others?

HOH is 62 I think, and Crypt of Decay I THINK is 55 as well? The rest of them require access.

Plane of Storms has a required level of 61 I think?
