In the House of Thule: The Executioner's Blade does reach into the east side of the SW room with the Beastlings of Horror along the east side if he is being pulled to the SW corner of the main room with the staircases. Also enrages.

In Sanctum Somnium Raid: The Executioner spawns as part of the first raid event in Sanctum Somnium, "Taking the Tower" (see this quest entry for event and loot information).
