A combination of the wide availiability of extremely good armor, and doctor buffs, has severely unbalanced the game. In order to make anything hard now, It must be given 200k HAM, heavy armor, and very high resistances.
All this will change with the combat revamp, which I believe is publish 11 or 12. In essence, everything is being nerfed. Armor is having the cap lowered, buffs will become less effective. I don't know if the difficulty of some enemies will be lowered to compensate.
